Middlesbrough grabbed an FA Cup quarter-final place after winning 5-4 in a penalty shoot-out against WBA after a 1-1 draw after 120 minutes.
Albion's January signing Sherjill MacDonald missed the vital spot-kick in sudden death as Boro booked a place in the last eight.
Boro's Dong Gook Lee and Albion's Neil Clement had also failed from 12 yards after the game had ended 1-1 after extra-time.
Jason Koumas floated a chip just over the bar as Albion were in command early on and after 25 minutes a packed Hawthorns erupted when Darren Carter put them in front with a low shot past a flat-footed Brad Jones.
But Mark Viduka (63) brought Boro back on level terms with a deflected shot off Curtis Davies.
The game went into extra-time and Nathan Ellington was given a red card for a challenge on Stuart Parnaby before the shoot-out drama.
